Despite recent progress in the treatment of small cell lung cancer, the prognosis for patients with advanced disease remains poor, and the majority of patients eventually succumb to their disease. Recently, the Charlson Comorbidity Index was validated as a prognostic variable in a variety of malignancies. The purpose of this study was to determine if a comorbidity index can be used in patients with small cell lung cancer. A total of 122 patients with small cell lung cancer were followed prospectively from January 1984 to December 1991. An index of comorbidity was calculated for each patient, and the survival of patients was analyzed using Cox's proportional hazards model. Independent risk factors for poor survival were age > 60 years, stage IIIB disease, weight loss > 10%, and extracranial metastases. The index included age > 60 years (P = 0.002), weight loss > 10% (P = 0.02), and stage IIIB disease (P = 0.007). The index was an independent risk factor for survival (P = 0.0002). This simple index of comorbidity is an independent prognostic variable for survival in patients with small cell lung cancer, and has the potential to improve the assessment of patients with this disease.Q:
